ON-LINE REGISTRATION

 

Online registration is available HERE. (NOTE - Attendees claiming student registration status cannot register via our online system. They must, instead, fill out and mail in a registration form available HERE. Registration forms claiming student status must be countersigned by the school program director of the student attendee.) PLEASE ADDITIONALLY NOTE- Student registration is intended for full-time students in schools of respiratory care, sleep medicine or nursing who do not already work for salary within those professions and who are not already credentialed in those professions. Student registrations that do not fufill that requirement will be rejected.

 

REGISTRATION BY MAIL

To register by mail, fill out a registration form which is available HERE. You will be sent to a screen that has the registration form in PDF format.

If all else fails, call us toll-free at 1-800-661-5690. We will either e-mail, fax, or mail a registration form to you as you request.

 

Please Note:

Early registration with discounted fees ends September 20, 2012. Registration continues after that date and on-site, but higher fees will apply.
